Job Description:
A company is looking for a Case Manager LPN (Licensed Practical
Nurse).
Key Responsibilities:
Maintain remote access to required EHR systems
Conduct data mining to close quality gaps within EHR systems
Contact provider offices to request medical records
Minimum Qualifications:
Active Licensed Practical/Vocational Nurse (LPN/LVN) license
At least 1 year of experience in provider outreach and/or EHR
system navigation
At least 1 year of experience with HEDIS /STARs data
abstraction
At least 2 years of experience in data analysis and quality chart
reviews
Proficiency in Microsoft Office applications, including Word and
Excel
