Job Description:
A company is looking for a Case Manager, Registered Nurse
(Remote).
Key Responsibilities
Assess, plan, implement, and coordinate case management activities
for members to evaluate their medical needs
Develop proactive care plans to enhance member wellness and address
identified health issues
Utilize clinical tools to evaluate member needs and conduct
assessments considering various health conditions
Required Qualifications
Active and unrestricted RN licensure in Louisiana or compact
licensure in state of residence
3+ years of clinical experience
2+ years of case management, discharge planning, or home healthcare
coordination experience
Associate's Degree in Nursing (required); Bachelor's Degree
(preferred)
