The Health Navigator is responsible for Member outreach and navigation support on issues of access and use of IEHP services and programs. Under the direction of the Manager of Community Behavioral Health & Social Supports and the Supervisor for the Health Navigator Program, the Health Navigator is proactive and responsive to Members needs in a friendly and professional manner. The Health Navigator helps individuals overcome barriers and engage in equitable and quality care. This experience is in addition to the minimum years listed in the Experience Requirements above.Four (4) years of experience in any/all of the following:Promotora/Community Health Worker experienceCommunity outreachCommunity event participationCustomer service experience in a related field such as public health, CHW work, teaching/education, or Case coordinationDelivering presentations/trainings to varied audiencesManaged care experienceBilingual preferredKey QualificationsMust have a valid California Drivers license and valid automobile insurance. Must qualify and maintain driving record to drive company vehicles based on IEHP insurance standards of no more than three (3) points.Be a proactive team player who is also able to work independently in assigned communities, and build rapport with diverse Members, providers, and local partner agencies.Knowledge of Medi-Cal/managed care environment required.
