Job Description:
A company is looking for a Local Contract Nurse RN - Case
Management.
Key Responsibilities
Facilitate the interdisciplinary plan of care focusing on medical
necessity and discharge planning
Coordinate activities to promote quality outcomes and patient
throughput
Identify potential barriers to patient throughput and implement
appropriate discharge plans
Required Qualifications
Recent acute care Case Manager experience within the last year
2-3 years of ICU experience or strong Med-Surg experience may be
considered
Experience in a Long-Term Acute Care (LTAC) facility is a plus
Active RN license
Ability to work 40 hours per week on an 8-hour shift schedule
