Job Description:
A company is looking for a Telephonic RN Nurse Care Manager.
Key Responsibilities
Manage a caseload of high-risk patients, coordinating care services
and developing personalized care plans
Facilitate transitions of care to prevent avoidable hospital
admissions and ensure patient needs are met
Provide education on disease management and health maintenance to
patients and caregivers
Required Qualifications, Training, and Education
Active Registered Nurse License
2+ years of care management experience in community, health plan,
or hospital systems
Strong clinical skills and proactive thinking
Knowledge of Medicare regulations and home care standards
Bachelor of Science in nursing or related field preferred
