Job Description:
A company is looking for a Family Nurse Practitioner (Virtual -
Temporary through December 2025).
Key Responsibilities
Conduct virtual wellness visits and health assessments for
patients
Collaborate with in-home team members to evaluate patient needs and
coordinate care
Provide comprehensive care management, including preventive care
and medication management
Required Qualifications
Board Certified as a Family Nurse Practitioner
Active Family Nurse Practitioner License
Exhibit high levels of professionalism, integrity, and
compassion
Technologically savvy and comfortable with digital tools for
patient care
Ability to work independently and adapt to a dynamic
environment
