Job Title: Outpatient Oncology Care Manager Profession: Registered
Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day Hours
per Shift: 8 hours Experience: 2 years of Case Manager experience,
experience with Oncology and Pediatric Neuro Oncology License:
Valid RN license required Certifications: PALS or PEARS required;
CPHON preferred Must-Have: - Oncology background - Experience with
Neuro Oncology patients - Knowledge of Chemotherapy and its
interactions Description: This role is for an Outpatient Oncology
Care Manager. The position caters to Neuro Oncology patients. The
nurse will work in a multidisciplinary team to develop
individualized care plans. Accountability includes patient
assessments, implementing physician orders, and evaluating care
processes. Coordination of follow-up appointments and ordering of
labs is essential. The nurse will manage referrals, authorizations,
and durable medical equipment (DME) coordination. Transition
planning and needs assessments are key components of the role. The
average daily census is between 20 to 22 patients. This position
does not involve administering Chemotherapy but requires knowledge
of its principles. Float coverage for other colleagues is required.
The candidate must be comfortable working with diverse age groups,
